AFFIDAVIT OF WITNESS

I, Mr. Kalvin Fontillas, age, civil status, resident of #7 New Era Street,
Central Avenue, Quezon City, after having sworn in accordance with law, do
hereby depose and state THAT:

1. I was present and personally assist Mr. Armando Ambong in processing

his loan to our company GALANTE LOAN AND INVESTMENT
CORP.

2. Affiant is the best friend of Mr. Armando Ambong and we’ve known
each other since we were kids so when he asks for financial assistant On
December 8, 2011 for his 2-year old daughter’s hospital bill amounting to
Php6,500,000.00 as much as I want to help him I do also have financial
constraints which is why I offer that he can loan to our company the
GALANTE LOAN AND INVESTMENT CORP where I am an
employee thereof.

3. On December 12, 2011, at 10:00 in the morning, Mr. Armando Ambong

went to our office and since I am the auditor-in-chief, I assist him. In
trust and in confidence I just relay to him verbally that since Mr.
Armando Ambong will loan such large sum of money, the interest of it
shall be waived provided that the he should continuously and religiously
pay the said amount within 3 years. I also inform him that he shall
received notices from the company every 29 th of the month and he could
 pay through online payments or bank transfers. Failure to do such verbal
conditions, he shall receive penalty from its outstanding payment of zero-
point two percent of penalty for everyday of delay of payment and legal
actions may be taken against him

4. Thereafter Mr. Armando Ambong agreed upon that he shall bound to pay
the monthly amortization amounting Php125,663.21 starting February 29,
2012.

I executed this affidavit to attest the truthfulness of the foregoing facts with
the best of my knowledge.
