PAST SURGICAL HISTORY: Urinary Catheter Insertion (June 2022)

MEDICATIONS: Not specified

CASE REPORT

The patient is a 54-year-old male who has a longstanding history of hypertension and diabetes
and presents with a complaint of pruritis, lethargy, lower extremity edema, nausea, and emesis.
In June 2022, the patient was diagnosed with renal failure and on the same year he undergone
Urinary Catheter Insertion.

On physical exam the patient is a well-developed, well-nourished male in moderate distress. In


KUB ultrasound, right kidney measures 9.9 x 5.6 x 5.9 cm while left kidney measures 8.1 x 4.3 x
4.0 cm which also indicates atrophic kidney since the average size of a normal kidney is 9-10 cm
but with our patient's left kidney measurements it is more likely the left kidney is atrophic kidney
which means it is smaller than its normal size. right kidney shows moderate hydronephrosis.
Creatinine level is elevated. Right proximal ureter is dilated.

CLINICAL INTERVENTION AND PROPER MEDICATION

Mostly for this kind of cases treatments depends on the underlying cause and controlling that,
plus alleviating symptoms and preventing complications are the focus of management.
Medications, fluid management, attention to calorie intake, dietary changes, dialysis and possibly

kidney transplantation are all frequently utilized.

CONCLUSION

The renal parenchyma is the functional part of the kidney that includes the renal cortex (the
outermost part of the kidney) and the renal medulla. Renal parenchyma disease describes
medical conditions which damage these parts of the kidney. These diseases may be congenital,
hereditary, or acquired.

Renal parenchymal disease is the most common cause of secondary hypertension, accounting for
2.5% to 5.0% of all cases. Hypertension associated with renal parenchymal disease occurs as a
complication of a wide variety of glomerular and interstitial renal diseases and may accelerate
the decline in renal function if inadequately controlled.
