SPECIAL ISSUE MYSTERIOUS HEPATITIS SUSPECT

INVESTIGATION : WHAT A RADIOLOGIST FOUND ?

Abstract
Background: Five viruses are responsible for most cases of viral hepatitis, which
is an inflammation of the liver due to a viral infection. These are
the hepatitis A virus (HAV), hepatitis B virus (HBV), hepatitis C virus (HCV),
hepatitis D virus (HDV) and hepatitis E virus (HEV). All the hepatitis viruses can
cause acute hepatitis. At the moment, Journal of hepatology have identified of
immune correlates in a case of mRNA vaccine-associated autoimmune hepatitis
(SARS-CoV-2 vaccination can elicit a CD8 T-cell dominant hepatitis) with a
unique pathomechanism associated with vaccination induced antigen specific tissue
resident immunity requiring systemic immunosuppresion.
Objective: To describe how to recognize and identification of mysterious hepatitis
supported by radiology approach.
Methods: Methods was design by case report (patient sample). A 46 years old male
confirmed after >30 days received second dose mRNA vaccine. There are
symtomps including, 3 weeks history of fever, oxygen saturation was 97%, night
sweats, jaundice, fatigue, and nausea-vomit reccurent.
Result: Performed by ultrasound : hepatomegaly >16 cm at the midclavicular line,
starry sky apperance, CT : hepatomegaly, possible diffusely decreased
parenchymal attenuation on non contrast CT, hepatic steatosis. MRI are non
spesific.
Conclusion: The fact that mysterious hepatitis suspect (correlates in a case of
mRNA vaccine autoimmune) may have similiraties criteria with a hepatitis due to a
viral infection(common) based on radiographic approach.
