GUERRA, of legal age, married, Filipino citizen, and a resident at Phase 2, Block 2 Lot 33, Diamond St., Celina Homes 1, Camarin, Caloocan City, after having duly sworn accord to law, depose and say: I am the owner of a 2017 Suzuki Ertiga with Plate No. NBK 3283 which I acquired last November 2017; Last June 05, 2022, at around 7 o’clock in the morning, as we were about to go to church, I noticed that our car was hit while it was parked outside our house. The left portion of the rear bumper, was damaged; Tam executing this affidavit for purposes of attesting to the truth of the foregoing, and for whatever legal purpose it may serve.
