NOW COMES the Plaintiff/Appellant HEXAGRAM HOME HEALTH CARE, LLC

(“HEXAGRAM”) by and through its attorneys, RAIZ ALMIRON LLC, and hereby provides

notice to all parties that it is appealing this matter to the First District of the Appellate Court of

Illinois from the Circuit Court of Cook County, Illinois from the order entered on May 22, 2012

by the Honorable Margaret Ann Brennan, a copy of which is attached and labeled as Exhibit A.

This appeal is from a final order of May 22, 2012, granting Defendant ILLINOIS

DEPARTMENT OF EMPLOYMENT SECURITY, an administrative agency in the State of

Illinois and DIRECTOR OF ILLINOIS DEPARTMENT OF EMPLOYMENT SECURITY’s

Motion to Dismiss Plaintiff’s First Amended Complaint.

 The relief sought is for this Honorable Court to reverse the order granting Defendants’

Motion to Dismiss and for any and all other relief as this Honorable Court deems fair and just.
