    Authorization to Perform Background Checks

    I , authorize HALLIBURTON GUYANA INC and/or the company


    defined by Halliburton to process and perform a complete background check.
    This process will include: (1) Government issued identification verification; (2)
    Criminal record check; (3) Sex offender registry where available; (4) Global
    sanctions list; (5) Motor vehicle record; (6) Employment verification for the
    previous jobs; and (7) Position qualification checks such as education
    (College/University) or equivalent experience and professional accreditation
    when is applicable.

    In order to perform the above listed processes, I authorize HALLIBURTON


    GUYANA INC and/or the company defined by Halliburton to collect, store, use,
    report, and disclose my personal and sensitive data.

    The information will only be shared to clients in compliance with commercial


    contracts.
