COMPLAINT-AFFIDAVIT

I, ALVAN A. MOHAMAD, of legal age, filipino, with home address at brgy. San Isidro
General Santos City, after being sworn to in accordance with law, hereby depose and
state that:

1. I am the father of Lance X. Mohamad as evidenced by his birth certificate

herein attached as “ANNEX A”,

2. I am executing this complaint-affidavit to charge Cavahug X. Bugan for

HOMICIDE defined and punished under Article 249 of Revised Penal Code
for the death of my son, Lance. At about 4:00 pm on January 14, 2023 the
offense was committed at Mabuhay Road, while Bugan was driving the car
above the limit and he hit a motorcycle drove by Lance.

3. After the accident Lance was rushed to the Elizabeth hospital and
pronounced dead on arrival (DOA) by the Doctor of the Elizabeth Hospital.
