Everest Rentals Private Limited een Gene ACCIDENTS / INCIDENTS / NEARMISS INVESTIGATIONS, IssueN0.01 Revision No.0 ise Dae owraza 1 Purpose: Handling Accidents, Incidents and near misses. 2 Scope: All areas of Operations and OHSAS concems. 3 Procedure: 3.1 MR impresses the employees about the importance to report accidents, incidents and near miss ‘cases and also the safety of self and other employees. 3.2 HOD reports the near miss cases, loss/equipment damage, first aid, medical treatment cases, NCs through incident reports to Managing Director and MR. 3.3 Death or serious injury incidents are immediately reported to the MD by Project Manager/ HSE. 3.4 Director ensures that each event is properly investigated and appropriate corrective and / or preventive actions are initiated. 3.5 Managing Director ensures that the investigation efforts are commensurate with the potential or actual harm by involving all HODs. 3.6 HOD ensures that no employee suffers any hardship as a result of reporting a non-conformance, ‘accident or incident. 3.7 While investigating, OHSAS coordinator interviews the witnesses, takes photographs, collects data and analyze until on adequate, comprehensive explanation is obtained. OHSAS Coordinator identifies the root cause with appropriate corrective and / or preventive actions. 3.8 Director with OHSAS coordinator’s assistance reviews proposed corrective and preventive actions with respect to actual and potential NC's and magnitude of problems through the risk assessment process prior to implementation, 3.9 OHSAS coordinator conducts safety meetings on a quarterly basis and discusses / reviews all incidents / near misses and amends safety procedures and guidelines wherever required. 3.10 OHSAS coordinator applies the lessons leamt from the investigations by focusing on the issues involved, rather than being restricted to specific action. 3.11 Where operations are not under the direct control of the organization, employees follow the reporting procedures laid down by the client. However, employees complete the incident / near miss reports and submit the same to the OHSAS Coordinator. 3.12 MR assesses all the Incident reporls and intimates to the statutory authorities, client or concemed parties, as the case may be. 3.13 OHSAS coordinator communicates the OHSAS performance reports to all employees by displaying the results in notice boards and then discusses it in staff meetings. 3.14 An employee who witnesses a NEAR MISS incident must complete the NEAR MISS reporting form (ERPL/HSE/FM/10) and submit to the Safety In charge. The reporting system is non-punitive and, if desired by the person reporting, anonymous. 3.15 All Nearmiss incidents will be reviewed by the Safety In charge and the Health & Safety Committee to identify the root cause and the weaknesses in the system contributing to the incident. All of these represent opportunities for training, feedback on Performance, and a commitment to continuous improvement, Communicatio: The steps taken (training, new policies & procedures, etc.) to improve workplace safely as a result of the NEAR MISS investigation will be reported to the Management by the Safety In charge. Monitoring and investigation results of NEAR MISS incidents will be communicated to employees in at least one of the following venues: + Safety & Compliance Manager discussion with employees. ‘+ Bulletin board postings; and/or + Minutes of Health & Safety Committee Meetings. Roles & Responsibilities: Its the responsibility of ERPL to: Enforce federal health and safety laws, standards and procedures. Suppor the development and implementation of safe working practices through provision of proper resources. Review health and safety procedures annually (minimum) or as necessary: and Consider incentives that encourage NEAR MISS reporting and enhance the culture. Its the responsibility of the Safety In charge to: Enforce federal health and safety laws, standards and procedures. ‘Acknowledge and document potential hazards reported by ERPL employees. Monitor health & safety performance, re-designing health and safety practices and procedures when prudent to do so. Include training for new employees to identify hazards and work safely as part of their orientation; and Celebrate the success and value of the NEAR MISS reporting process with all employees! Promptly report (within 24 hours) all incidents by completing the NEAR MISS REPORTING form, (reporting only FACTS) and submit to Safety In charge; and Provide statements and participate in the NEAR MISS investigation. Itis the responsibility of Outside Contractors to: Follow the same reporting procedure as employees directly employed by ERPL; and Provide statements and participate in the NEAR MISS investigation. Non-Punitive Exemptions:- NEAR MISS reporting is non-punitive and workers will not be subject to progressive disciplinary measures unless their behavior coincides with one of the following serious offences: Willful breach of professional codes. Acts of gross negligence. Acts of gross misconduct (e.g. Possession of alcohol. illicit narcotics or non-prescribed pharmaceuticals while on company property, or use thereof while operating ERPL equipment) Repeated unreported violations Malicious activities (including malicious reporting of untrue allegations against a colleague}: and/or Workplace violence, including but not limited to fighting, assault, harassment or possession of a weapon. 5.0.
