APPLICATION FOR A CHANGE OF NAME

IN THE MALAYSIA PASSPORT

No. Document(s)

1. Covering letter, briefly stating reason for change of name.

2. Approval letter on the change of / additional name obtained from the

Identity Card Division, National Registration Department, Putrajaya.

3. Copy of Malaysian Identity Card (IC) / Temporary IC KPPK 09.

4. Copy of residing proof in Singapore, e.g. Singapore IC for PR, E.P etc.

5. Copy of birth certificate

6. Copy of Statutory Declarations for declaring on the change of name.

(This service is available at the Consular Section at the High
Commission.)

7. Other relevant documents, such as the marriage certificate, certificate

of baptism, certificate of religious conversion etc. (where applicable).

Note:
a) Applicant will be issued with a new passport.
b) Application will be sent to the Headquarters of Immigration
Department of Malaysia, Putrajaya for processing and approval.
Processing period will be between 2 – 4 weeks.
