This is to permit my child Angel Rosa to perform the data gathering in his/her
chosen locale in the subject Research Project under Mr. June B. Mijares specifically in
Sto. Nino National High School, Sto. Nino, Sta. Maria, Romblon on February 22, 2023.

I fully understand that my child together with his/her groupmates were the ones
who decided and chose the said locale;

I fully know that the subject teacher briefed and reminded his students of the
risks and precautionary measures when his students including my child decided to
choose locales outside the school premises;
