ADMITTING ORDERS

o Please admit under my service to room of choice

o Secure consent for admission

o Diet: NPO at 6am January 22, 2023

o IV Fluids: PNSS 1L 12h

o Diagnostics: Attach to Chart (Done Out Patient Basis)

o Attach CP Clearance done to chart

o Pre Op Orders: Patient is for Percutaneous Nephrolithotomy Right tomorrow 2:00PM

o Secure Consent for admission.

o Inform OR. Secure Lap Tower and C-ARM for OR use

o Medications:

1. Ceftriaxone 2gm TIV as loading dose 1 hour prior to OR ( ) ANST

2. Gentamycin 40mg TIV now then secure 1 vial for OR use
3. Tranexamic Acid 1gm TIV 1 hour prior to OR
4. Omeprazole 40mg TIV OD while on NPO

o Please refer patient to: Dr. Miguel Dellosa for Anesthesia

o Refer accordingly
