Statement of the Problem: An Evaluative Study to Determine the Effectiveness of Nurse Led
Educational Program on Oral Anticoagulation Therapy upon Self efficacy among Patients with
Cardiac Disease at Selected Hospitals, Chennai

Objectives

Primary Objectives
1. To assess the knowledge and adherence of oral anticoagulation therapy among patients with
cardiac disease.
2. To determine the effectiveness of nurse led educational program on oral anticoagulation
therapy on self-efficacy among patients with cardiac disease.
Secondary Objectives
3. To assess the level of acceptability regarding nurse led educational program among patients
with cardiac disease receiving oral anti coagulation therapy in experimental group.
4. To determine the correlation between the knowledge and adherence of oral anti coagulation
therapy among patients with cardiac disease in control and experimental group.
5. To determine the association of selected demographic variables with self-efficacy of oral
anti coagulation therapy among patients with cardiac disease in control and experimental
group.
6. To determine the association of selected clinical variables with self-efficacy of oral anti
coagulation therapy among patients with cardiac disease in control and experimental group.

Operational Definitions

Effectiveness: Refers to the possible outcome in terms of improvement in knowledge and


adherence of patients after the administration of nurse led educational program on oral
anticoagulation therapy among patients with cardiac disease.

Nurse Led Educational Program: Refers to a structured educational program designed by the
researcher for patients with cardiac disease who are on anticoagulation therapy. This includes
drug and dietary interactions such as vitamin K-rich foods, safety with herbal therapy, the need to
watch for signs and symptoms of bleeding, risk associated with trauma, the need for intensive
follow up and drug monitoring and duration of therapy. The program will be conducted by lecture
and discussion for 1 hour using ppt, video and handouts.
Oral Anti Coagulation therapy: Refers to the class of drugs given for patients with cardiac
disease to treat the thrombotic disorders by suppressing the synthesis or function of various
clotting factors in the blood vessel.

Knowledge: Refers to the patient’s level of understanding regarding anticoagulation therapy as


measured by The Anticoagulation knowledge Tool (AKT) developed by Obamiro, 2016.

Adherence: Refers to the patient's agreement with the recommendation from a health care
provider regarding oral anti coagulation therapy as measured by The Modified Drug Adherence
Work-Up (M-DRAW) developed by Lee, 2017.

Patients with Cardiac Disease: Refers to the group of patients suffering from heart and blood
disorders and who are on oral anticoagulation therapy as diagnosed by the cardiologists.

Research Hypotheses
H1: There is significant difference in pre-test and post-test self-efficacy on oral anti coagulation
therapy between control and experimental group of patients with cardiac disease.

H2: There is significant correlation between knowledge and adherence of oral anti coagulation
therapy among patients with cardiac disease in control and experimental group.

H3: There is significant association of selected demographic variables with self- efficacy on oral
anti coagulation therapy among patients with cardiac disease in control and experimental group.

H4: there is significant association of selected clinical variables with self- efficacy on oral anti
coagulation therapy among patients with cardiac disease in control and experimental group.

Research Methodology
Research Approach: Quantitative approach
Research Design: Quasi Experimental Research design
Dependent Variables: Self-efficacy (knowledge and adherence) of oral anticoagulation therapy
among patients with cardiac disease.
Research Setting: Apollo hospital, Greams road and Apollo specialty hospitals, Vanagaram,
Chennai
The Target Population: Patients with cardiac disease on oral anticoagulation therapy.
The Accessible Population: Patients with cardiac disease at selected hospitals, chennai
Sample Size: Sample size was calculated using Open Epi based 104 divided into 52 in control
and experimental group each.
Sampling technique: Two setting will be randomly selected and samples from one setting will be
taken for the control and the samples from another setting will be taken for experimental group.
Data Collection tools:
 Demographic variables proforma of the patients with cardiac disease
 Clinical variables proforma of the patients with cardiac disease
 The Anti coagulation knowledge Tool (AKT)
 The Modified Drug Adherence Work-Up (M-DRAW)
 Rating scale on level of acceptability of nurse led education program on anti-coagulation
therapy
Protection of Human Rights

 Permission will be obtained from the principal, HOD of Medical Surgical Nursing, Head
of Research and Development.
 Institutional Ethical committee clearance will be obtained.
 Setting permission will be obtained from the concerned authorities of the hospital.
 Purpose of the study will be explained to the participants.
 The informed consent will be obtained from all participants, after explaining the purposes
of the study.
 Confidentiality and privacy of study participants information will be maintained
throughout the study.

Plan for Data Analysis: Descriptive statistics (frequency, percentage& standard deviation)
and inferential statistics (Paired ‘t’ test, chi square, Pearson correlation,).
