ALTERNATIVE HOSPITAL QUARANTINE GUIDELINE Alternative Hospital Quarantine at Bangkok Hospital Pattaya: Alternative Hospital Quarantine (AHQ) program is now available at Bangkok Hospital Pattaya for those who seek medical treatment are currently living outside of the Kingdom of Thailand. Presently, patients who would like to apply for the AH@ program are required to be quarantined in the Isolation Ward in the hospital at least 14 days (the first day of arrival is being counted as day “O”) while having their treatments as an in-patient (depends on your physician's recommendation). The patient must complete the hospital quarantine without any exceptions during the first 14 days. Hotel quarantine is deemed impractical for patients who seek medical treatments in Kingdom of Thailand ~ and the real-time PCR for COVID-19 (SARS-CoV-2) is required to be performed on 3 occasions. Requirement for non-Thai nationals prior to traveling into the Kingdom of Thailand for Medical Tourism: 1. Avoid travelling into a COVID-19 affected area or communities at least 14 days before travelling. 2. Required documents: 1 Certificate of Entry 2) Fit For Travel Medical Certificate 3) Certificate of COVID-19 Free (issued not more than 72 hours before the flight). 4) Documents ensuring the ability to cover health and all the expenses. 5) Health insurance covering COVID-19 (not less than 100,000 US dollars). 6) Letter certifying the need for treatment from the medical institute of the country of origin, 7) Letter of Confirmation (in order for the patient to apply for the VISA and Certificate of Entry (COE). 8) Invitation letter from your preferred hospital in Kingdom of Thailand. 9) Letter of approval to be hospitalized and quarantined not less than 14 days from the hospital. 10) Official approval of Alternative Hospital Quarantine from Ministry of Public Health of your preferred hospital. 3. If the treatment is less than 14 days, mt ALTERNATIVE HOSPITAL QUARANTINE GUIDELINE Process of getting the VISA and Certificate of Entry (COE) for medical tourism purposes: 1. Online inquiry and online consultation with the physician via inquiry@bph.co.th. 2, Patient receives the recommendation letter for the treatment(s) - including length of stay for the treatment(s) - and full information of the Alternative Hospital Quarantine (AH@) program from Bangkok Hospital Pattaya (BPH). 3. The patient agrees to the terms and conditions and provides the following information in order to receive the Letter of Confirmation and Invitation Letter from BPH: + Confirmation of the treatment date on which the patient wishes to undergo the treatment(s). + Copy of the patient’s passport. * Documents ensuring the ability to cover health and all the expenses (summary of bank statment from the past 3-6 months or confirmation of letter of payment from the third party). 4, BPH provide the Letter of Confirmation and other required documents for the patient. 5. The patient apply for the VISA and the COE at the Thai Embassy in his/her country by submitting all the required documents. However, the patient can submit the flight confirmation, the COVID-19 undetected test result, and Fit to Travel Certificate after receiving the COE from the embassy - less than 72 hours before the flights. Thereafter, the patient must submit the required documents to BPH at least 72 hours prior to his/her arrival time into the Kingdom of Thailand. Letter of Confirmation (in order for the patient to apply for the VISA and Certificate of Entry (COE). 2. Invitation Letter to be hospitalized and quarantined not less than 14 days from the hospital. 3. Official approval of Alternative Hospital Quarantine from Ministry of Public Health of your preferred hospital 4, Letter of Approval to be hospitalized and quarantined not less than 14 days from the hospital (Once receives patient's flight confirmation and COE), 5. Confirmation letter of the AHQ program (once the patient has completed the AHQ program). 6. Medical report (once the patient has completed the AH@ program). Protocol for Alternative Hospital Quarantine (AHQ) at Bangkok Hospital Pattaya (BPH): + Patient and companion(s) will be subject to quarantine at BPH for the full duration of at least 14 days - from the day of arrival into the Kingdom of Thailand (the first day of arrival is being counted as day “o") + Body temperature and respiratory symptoms will be monitored on daily basis (at least twice a day). + Nasopharyngeal and oral swabs for real-time PCR for COVID-19 (SARS-CoV-2) will be performed on 3 occasions; on day 0, 5-7 and 14. + The patient will only allow proceeding with treatment(s) as planned, once the first COVID-19 (SARS-CoV-2) result is confirmed undetectable. « If the COVID-19 (SARS-CoV-2) test result is detectable, the infected individual will be cared for in BPH until the patient complete all of treatment(s) as advised. + Patient care at BPH will be provided under the supervision of the patient's physician(s) together with Infection Control Specialist throughout hospital quarantined. + Patient is only allowed to leave the room for medical consultations) and treatment(s) as planned while the companion must remain in the room throughout the quarantine period of 14 days. + Patient is allowed to have maximum of 3 companions and those must be the patient's spouse and immediate family only. + Only the first companion is allowed to be quarantined in the same room as the patient. * The second and third companions will be quarantined in the isolation room, and the room cannot be shared between the second and third companions (two separate rooms are required). + The second and third companions are only allowed to visit the patient twice a day: one hour in the morning and one hour in the afternoon. The damage or loss of items must be reported within 24 hours after you received the cleaned items. The company will take no responsibility on any items reported after 24 hours. 4. If you do not receive your items within 3 days and without informing the ward staff, the company will take no responsibility for those items. 5. The company reserves to provide laundry services only for the types of fabrics specified in the table only.
