AFFIDAVIT OF LOSS

 I, CHERRY MAINE H. GONZALES, of legal age, Filipino, married and

a resident of #4 INTERIOR 6, M. DELA CRUZ, KRUS NA LIGAS, QUEZON
CITY, after having been sworn to in accordance with law, hereby depose and say
that:
1. That I bought a Refrigerator (FUJIDENZO INR-82S) at SM North
Appliance Center at around December 2021;
2. That on January 29, 2022, when I cleaned the refrigerator, it suddenly
stops working;
3. I asked the customer service of SM North Appliance Center for warranty,
but they are looking for the official receipt of the refrigerator;
4. I tried to look and find the official receipt, but despite diligent effort to
locate the same proved futile;
5. That in view thereof, I now believe that they are lost beyond recovery;
6. That I am executing this affidavit to attest to the foregoing facts and for
any legal purpose it may serve.
