    CERTIFICATION

    This is to certify that the transportation expenses being requested by (name


    of employee) for his travel on (duration of travel) at (destination or venue) under
    Travel Order No._______ from (address of residence or permanent official station)
    to (destination) and back is for reimbursement in lieu of the prescribed fare using
    the public vehicle.

    This is to further certify that the reimbursement of personal use of vehicle is


    limited to the round trip cost of the customary mode of transportation.
