Case Analysis:
A 50-year old female came into the clinic complaining of moderate to severe back and
shoulder pains. She is married for 25 years and without a child. She works as an
administrator of a university. Recently, she defines her environment as stressful as
she rendered overtime to finish paper works. Her husband used to be supportive of
her, but she noticed that they have arguments lately resulting to misunderstanding.
Because of this, she is having difficulty in sleeping at night and chest discomforts.

What specific nursing interventions will you render to the client to conserve her
health?
Energy Conservation
The client is experiencing mental stress due to her stressful working
environment as she rendered overtime to finish paper works. She is also experiencing
difficulty in getting a good night’s sleep.
Suggest to the client to practice time management.
Suggest activities like taking a warm bath and drinking warm milk before
bedtime to improve the quality of sleep.
Encourage her to establish a bedtime routine.
Introduce relaxing activities like self-meditation, imagery, or journaling to
distract attention away from the stress.

Structural Integrity
The client is suffering from moderate to severe back and shoulder pain, and
also having chest discomfort.
Encourage the client to utilize relaxation techniques to alleviate the pain.
Suggest to wear uncomfortable undergarments to somehow relieve chest
discomfort and promote rest and comfort.
Guide the patient to execute regular breathing exercise to improve chest
pains/discomfort.
Administer medication (pain reliever) as prescribed by the Doctor.

Personal Integrity
She is experiencing loss of selfhood or quality time for herself.
Recognize the client’s need for personal space.
Empower the client for her to regain selfhood.
Offer options to self-meditate/reflect.

Social Integrity
The client is having emotional stress due to the problems between her and her
husband. She is experiencing lack of support system from her husband and frequent
arguments resulting to misunderstandings.
Encourage the client and her husband to have a heart-to-heart talk to resolve
their issue.
Educate the husband of the client about the current situation of his wife for him
to understand her better and regain his support system for his wife.
