Step by Step Procedure to create HRP


Pre-requisite for HPR Creation

• Documents Required for Healthcare Professional Registry (HPR)


– Aadhaar card
– Degree Certificates with award date and month
– Council Certificates with award date and month
– Work details documents
• HPR Registration has two parts
– HPR ID Creation section (aadhar card with registered mobile Number is required)
– Detailed documentation upload section (Personal Details, Registration/council details, Work Details)
• Document Verification, Council Verification and Work Details verification will be
done

Step by Step Procedure to create HFR


Pre-requisite for HPR Creation

• Documents Required for Healthcare Facility Registry (HFR) / Facility Manager


– - Aadhar card
• Creating of Healthcare Facility
– - Hospital Details (Photo of Institution, PME Certificate, Timings, Services Available)
• Facility can be created with existing HPR ID or can go with creation of Facility Manager
– Facility manager is one of the Health care professional on non technical based administrative managers of
the institute.

HFR Advantages
• After successful facility registration, User id & password for
facility will be sent to register mail for generating ABHA cards
through Biometric process.

• ABDM has 3 Components
– ABHA (Ayushman Bharath Health Account)
– HPR (Health Care professional Registry)
– HFR (Health Care Facility Registry)
• ABHA Card can be generated in two ways
– self mode ( self registration / creation through ABHA web portal)
– assisted mode ( registration at Health facility)
Road Map to implement ABDM in state
• District Health Officers are the Nodal officers cum Verifiers of HPR and HFR
of the District.
• District Health and Family welfare officer will be the facility manager of the
district to verify HFR
• District Leprosy officer will be the Nodal officer to Generate ABHA cards to
the population of the District.
• All District Programmee officers need to take lead in generating ABHA
cards with their respective program and cover the population, as all GOI
programs are ABHA generation compliant.
