ASSESSMENT: Practical Action – Mental & emotional wellness (Weighting 30%)

Purpose
To participate in, and evaluate, a health-promoting activity beyond the classroom.

CHOOSE A S.M.A.R.T. GOAL: What do you want to achieve?

PRE-TEST: Conduct some form of test to determine a starting point for improvement.

TWO WEEKS: Go about your activity and monitor your progress with a journal

POST-TEST: Conduct the same test as before to monitor any improvement

Description of Assessment
This practical activity gives you the opportunity to work towards achieving a health-promoting goal. The
activity requires you to:

 identify a personal goal to improve an aspect of your personal health and well-being

 supported by research and evidence, explain how the goal you have set for yourself helps achieve
well-being for you and/or others

 contact and interact with an individual or organization that can assist you to achieve your goal OR
include research evidence as to the benefits of this goal. Keep a record of your contact and
progress for evidence. These details need to be evident in your reflection and presentation of the task

 plan what you intend to do and how you intend to achieve this. Comment on the processes and
outcomes involved (observations, interactions) and the strategies put in place to achieve this goal

 reflect on how successful you have been and how you can assess this (e.g. What worked well? What
was not as successful? What would you do differently next time?).

 comment on how sustainable this activity is, and on the long-term impact of setting this goal.

Assessment conditions
Evidence of your participation, including observations, interactions, and reflection, may be presented in
written, oral or multimodal form. May include pictures and diagrams of your action.

 How will you be measuring the
success of your contract?

Background Research:

 Explain your issue in society. Provide research into what the problem is, who it affects, the health implications associated
with it, trends, statistics etc.

Interview (optional):

 Who are you going to interview?


 Why did you choose them?
 What advice did they provide?
 Maintain a copy of your interview as a transcript

Plan:

 What are you going to do to achieve your goal?


 Discuss the strategies you will undertaking over the two weeks
 If possible, produce a table identifying your plan over time.

Process:

 Discuss the journey over the two weeks.


 How did you feel while conducting your practical action?
 What were some of the challenges you faced?
 What were some of the successful moments?
 Using emotive and descriptive words to describe your process
 Document this information during the process of your contract.

Reflection:

 How successful were you in making a change to improve physical wellness?


 Discuss the improvement (or not) from undertaking your action
 What would you do differently next time?
 Is your activity sustainable?
 What would be the health benefits from continuing your activity long-term activity?
