NOTICE
PHILIPPINE MILITARY ACADEMY CLASS OF 2027 CADET CANDIDATES
Congratulations!
The Philippine Military Academy would like to congratulate you for passing the
PMA Entrance Examination 2022.
You have been considered as a Cadet Candidate to undergo the next phase of
the selection process for cadetship to the Philippine Military Academy (PMA) which is
the Complete Physical Examination (CPE) that will be conducted at the Armed Forces
of the Philippines Health Service Command (AFPHSC). Should you be interested to
pursue your application, you are directed to report at the PMA Cadet Control Liaison
Office (PMACCLO), AFPHSC, V. Luna Road, Quezon City not later than seven
o’clock in the morning (7:00AM) on 06 January 2022 (Friday). The CPE shall last
for ten (10) days which you will be temporarily confined within the premises of the
AFPHSC and PMACCLO facilities for a 3-day quarantine and 7-day medical
examinations and a physical fitness test. Lodging will be at the PMACCLO for the
entire duration. Bring this notice when you report for the CPE, together with the
following:
a. Local Clearances:
1. Barangay Clearance with photocopy
2. PNP Clearance with photocopy
3. NBI Clearance with photocopy (If available)
4. Court Clearance or Regional Trial Court (If available)
5. Mayor’s Clearance
b. Recent Valid ID (School ID, Driver’s ID, Passport, etc.)
c. COVID-19 Protocol Requirements:
1. Vaccinated Cadet Candidates: Vaccination Card – Primary Series
(1st and 2nd Dose) and Booster series (if applicable)
2. Vaccinated or not vaccinated Cadet Candidates: COVID-19
NEGATIVE Rapid Antigen Test Result valid within 24 hours of
reporting
d. PSA copy of birth certificate [One (1) original and two (2) photocopies].
e. Original copy and one (1) certified true copy of your High School
Diploma (Certified true copy will be taken by OCRA)
f. Certified True Copy of High School Form 137/138 for High School
graduates (and current Transcript of Records for college students) and
for ongoing Grade 12 students – High School Form 138.
[One (1) original and two (2) photocopies]
g. Parents/Guardian’s Consent (4 original copies duly notarized)
h. Ten (10) pcs of 2” x 2” and Four (4) pcs of 1” x 1” ID pictures (colored
with white background)
i. Clothing good for 10 days, WHITE T-shirt and denim long pants
j. One (1) pair running shoes with white socks (5 pairs)
k. Six (6) sets of athletic uniform (Preferably white T-shirt with short pants
for males and for females jogging pants or short pants with cycling
shorts)
l. Facemasks (30 pcs) and hand sanitizer/alcohol.
m. Toiletries
n. Beddings (pillow, blanket, mosquito net, etc.)
Read the important reminders regarding your requirements. In addition, you will find
also at the back of this page the physical exercises that will be administered during the
Physical Fitness Test (PFT). Please be advised to prepare for this accordingly.
Finally, attached is the Reply Slip. Fill in all appropriate entries on the Reply Slip and
send it immediately through the fastest means available to the address, PMA Candidate
Control and Liaison Office (PMACCLO), AFP Health Service Command, V. Luna Road,

IMPORTANT REMINDERS
1. Before reporting for the CPE proper, you shall be required to undergo Triage
at the PMA Candidate Control and Liaison Office, AFP Health Service Command, V Luna
Road, Quezon City. In preparation for this, please ensure that you possess the following
qualifications:
Any discrepancy on your part on any of the stated qualifications shall warrant your
immediate disqualification.
b. Push-Up
Minimum Passing Score:
Male: 15 repetitions
Female: 5 repetitions
c. Sit-Up
Minimum Passing Score:
Male: 20 repetitions
Female: 15 repetitions
