3.

0 CATARACT CASE PRESENTATION

My patient, a 70 year old male, ABC, resident of Narhe, Pune, and a farmer by occupation;
came with the chief complains of

Diminution of vision in left eye since 3 months

He was apparently all right about 3 months ago, when he started experiencing painless diminution of vision in the left
eye
It was gradual and progressive.

There was no H/O usage of glasses, foreign body sensation, trauma, glare and photophobia

He is a known diabetic, since 10 yrs. which is controlled by medication.


He has undergone cataract extraction with IOL implantation in the right eye about 10 months ago.

He is a vegetarian, has no addictions, and has a normal sleep pattern.

There is no significant family history

On Systemic examination
Patient has a
Pulse rate of 85 beats per min.
BP of 130/86 mmHg
On evaluating RS, CVS and P/A, no abnormality detected

He is conscious, well oriented to time, place and person

Ocular examination

Visual acuity:
RE LE
unaided 6/6 6/24
With pinhole 6/6 6/18
Head posture- normal

Face is symmetrical

Forehead shows presence of senile wrinkles

Eyebrows are on same level and are normal

FOR RIGHT EYE

Normal position of eyelids


Blinking rate is 12 blinks per min

Conjunctiva-
Bulbar conjunctiva- transparent and normal
palpebral conjunctiva- pink coloured and normal

Sclera-
Previous surgical scar visible at 12'O clock position

Cornea
Normal in size and shape
Arcus senilis visible
On doing window reflex test, surface is smooth
It is transparent and sensations are present in all 4 quadrants

Anterior chamber is of adequate depth


No abnormal content seen

Iris is brown coloured and normal pattern seen

Circular, Single pupil, at centre seen which is normal in size


Direct and indirect light reflexes are present

Lens- normal position


shining reflex seen suggestive of Pseudophakic

Eyeballs are symmetrical, normal size, normal extraocular movements in all quadrants

Digital IOP is normal

Regurgitation test is negative- on pressure over lacrimal sac area


FOR LEFT EYE

Normal position of eyelids


Blinking rate is 12 blinks per min

Conjunctiva-
Bulbar conjunctiva- transparent and normal
palpebral conjunctiva- pink coloured and normal

Cornea
Normal in size and shape
Arcus senilis visible
On doing window reflex test, surface is smooth
It is transparent and sensations are present in all 4 quadrants

Anterior chamber is of adequate depth


No abnormal contents seen

Iris is brown coloured and normal pattern seen

Circular, Single pupil, at centre seen which is normal in size


Direct and indirect light reflexes are present

Lens- in normal position,


Greyish white opacity seen
Iris shadow test- present

Eyeballs are symmetrical, normal size, normal extraocular movements in all quadrants

Digital IOP is normal

Regurgitation test is negative- on pressure over lacrimal sac area

Provisional diagnosis is - Left Eye Immature Senile Cataract, right eye Pseudophakic with diabetes.
