REQUIREMENTS FOR ORIGINAL APPLICATION FOR AUTHORITY TO OPERATE AS

AN INTERNATIONAL / DOMESTIC AIRFREIGHT FORWARDER

1. Verified Application in prescribed CAB form (with Cashier’s stamp of payment)

NOTE:
a. Filing fees:
International: Php14,544.00
Domestic: Php10,180.80
International and Domestic: Php24,724.80

b. Notice of Hearing (signed by the ACASFAD Chief) shall be issued to applicant

and should be published at least once a week for two (2) consecutive weeks in a
newspaper of general circulation.

2. Affidavit of Publication (proof that the Notice of Hearing was published)

3. If Corporation (includes One Person Corporation): Certificate of
Registration/Incorporation from SEC & Articles of Incorporation; If Sole Proprietorship
or Partnership, Registration of Business Name with the Department of Trade & Industry
4. If Corporation (excludes OPC), Latest General Information Sheet; If Sole Proprietorship,
Partnership or One Person Corporation, bank certificate(s)

NOTE: Paid-up Capital should be at least Php2Million; If Sole Proprietorship,

Partnership or OPC, bank certificates showing at least Php2Million;

5. Certificate of Training in Dangerous Cargo of at least one (1) employee or Certificate of

Enrolment and Copy of Official Receipt of Enrolment Fee of employee to undergo the
Dangerous Cargo Training Course subject to the submission of the Certificate of
Completion within three (3) months from date of issuance
6. Insurance Policy covering Office/Property (i.e. Fire, Comprehensive General Liability,
Property and All-Risk, etc.) and the corresponding proof of premium payment made
issued by the insurance company
7. Audited Financial Statements or Audited Pre-Operating Financial Statements

 Only applications with complete and valid documentary requirements shall be received
by the Board. The application shall be given due course by this Board within three (3)
working days from the completion of all the requirements, including the requirements of
publication and ocular validation.

Note: All documents are required to be certified as authentic by applicant and are subject to comparison with
the originals. 1% legal Research Fee included, as mandated by Pres. Dec. No. 1856, dated 26 December 1982
