APPLICATION

1. I am currently enrolled at the Philippine Christian

University – College of Law (hereinafter, "PCU College of
Law") at 1648 Taft Avenue cor. Pedro Gil, Malate, 0907 City of
Manila;

2. I have either satisfactorily and successfully completed

all my third-year courses or, by the end of this academic year,
I shall have completed all my third-year courses;

3. I will enroll in PCU College of Law’s clinical legal

education program for the ____________ term of Academic
Year 2022 – 2023;

 4. I am aware of, and will abide by, the limitations under
Section 4, and the duties and obligations under Sections 6 – 8
of Rule 138-A of the Rules of Court;

5. I have paid the necessary fees under Section 21(1) of

Rule 141 of the Rules of Court for the issuance of the Level 2
Certification as evidenced by an Official Receipt annexed in
this application.1

PRAYER

WHEREFORE, it is respectfully prayed that this application be

duly endorsed to the Office of the Court Administrator as provided
in Section 5 of Rule 138-A for the approval and issuance of a Level 2
Certification in my favor.

Other reliefs just and equitable are likewise prayed for.
