1. Full time availability (12 hour Days/ nights/ weekends) (schedule to be confirmed –likely DDNN 5 days off)
2. Part-time availability: Are you available for at least 4 shifts per every 2 weeks (12 hours/ days/ nights (M-F)/ 4
out of 6 weekends)?
5. Are you a relative of a current Trillium Health Partners Employee? If you responded YES, please indicate the
name and relationship to the Trillium Health Partners employee.
9. If you previously worked for Trillium Health Partners, please indicate department and role.
11. Are you applying for a Registered Nurse or Registered Practical Nurse position?
