Surgery/Endoscopy Observation Experience

1. What symptoms/health issues did the patient have that brought them to the hospital to

have the procedure? Describe the procedure and purpose.

The patient I chose to do my paper on was a 29 year old women who had a bunch of

problems prior to coming into surgery today. She came in to have a laparoscopy and possible

laparotomy right oophorectomy and removal of both tubes. She had already said she was not

planning on having children in life which is good because it probably would not be possible after

this surgery. I saw everything from beginning to end. We went in and got her and spoke with her

before her surgery and the nurse asked her if she had any concerns or questions about the

surgery. The nurse also looked in the chart for her pregnancy test results which were negative.

We took her back and Dr. Cavari came in and did the surgery in under 30 minutes. She had a

5.5cm right adnexal mass. Her symptoms that brought her into the hospital was pelvic pain. The

surgery before this one was when they removed one of her ovaries.

2. What patient preparation was done and why? Include any preparation that was done the

day before the procedure as well as the day of.

The patient preparation that was done included obtaining a pregnancy test. The nurse had trouble

finding the results and the patient said she never took one but they actually did this through a

blood sample. The nurse also obtained her history and found out she had a history of blood clots.

This was useful information because during surgery they put the PCDs on her legs to prevent any

clotting during surgery. Before the surgery, a negative covid test was also obtained because she

was not vaccinated.


3. Explain what you observed in the room during the procedure? What were the roles of the

various health care providers? How was the communication?

During the procedure, I saw every person in the room performing a specific task at all times. No

one was standing around not doing anything. I saw the surgical techs set up the surgical field and

they were also the ones handing tools to the doctor. The doctor was the one actually inside the

patient. She cut the tubes, ovary, and mass with a trochar with a blade on the end (something a

lot of the doctors now a days don’t use anymore). The nurse did the documentation during the

surgery and also helped out when needed if the doctor needed anything. The first assist was

always the doctors right hand man. They were also the ones who stitched up the patient

afterward. And finally, the CRNA who administered the anesthesia and monitored the patient the

entire time during surgery as well as after when they woke up.

4. What did the nurse do before, during and after the procedure that protected the patient

from harm? How did the nurse act as the patient advocate? Include answers from a

nursing perspective (as if you are the nurse in the room for the procedure).

The nurse played a vital role before, during, and after the procedure that protected the patient.

They provided cushion and padding to the arms to protect them from falling off the bed during

surgery and also in case someone were to accidentally knock into them. The nurse ALWAYS

counted the materials before and after surgery and had a written count of everything. They also

went and got the patient and got signatures from the patient for documentation purposes. The

nurse acted as an advocate during surgery when she stopped the team from going any further to

go over the patient history and the procedure they were doing to make sure it was correct.
