Scenarios:

1 – Post OP Bed
“While Mr. Manansala is still in the recovery room, I will do the Post Op Bed. It is a bed
for those who had undergone surgery and for them to have a safe and comfortable bed
after surgery that protects the bed or mattress from being wet and soiled. This also
ensures that the equipment that the patient needs after surgery is already assembled at
the bedside.”
2 – Intradermal Injection
“Prior to the administration of medication, first we have to do an intradermal injection to
test for possible allergic reaction. This is to identify allergens to which the patient may
be hypersensitive by the means of skin testing using a syringe and needle into the
superficial layer of the skin. A wheal will appear and the doctor will examine the site.
3 – Vital Signs
“The next thing to do is to get the patient’s vital signs. Checking of vital signs will
indicate the state of the patient’s essential functions. These essential signs can be
observed and gotten from our patient and this will determine whether the condition
improves or if there is something wrong with the patient”
4 – Perineal Care and Occupied bed
“First is to perform perineal care to avoid infection from the soaked diaper and to
promote comfort to the patient then proceed to do occupied bed to maintain a hygienic
environment and reduce transmission of microorganisms.”

5 – Give a Sitz Bath


“We have to give the patient a hot sitz bath to promote comfort, increase local
circulation thus reduce swelling and promote healing from the surgery. This also helps
relax local muscles and relieve pain caused by the surgical excision of the
hemorrhoids.”
6 – Post Mortem Care
“The next nursing action is to perform post mortem care to prepare the body in a
manner that will reduce the family’s distress when viewing the body and prevent
distortions in the body’s appearance since the body will experience physiologic
changes.”
7 – Perineal Care
“My nursing action would be to do perineal care to the patient to clean the area of
secretion and excretion from the perineal or catheter, prevent skin irritation and
infection, and to promote comfort which can help the patient to void easily.”
8 – Medical Handwashing
“The first thing to do is to perform medical handwashing. This is to prevent the spread of
microorganisms, diseases, and nosocomial infections.”
9 – Cold Compress
“The best initial nursing action is to apply cold compress to the sprained right ankle.
This is to decrease the swelling, provide topical anesthesia, and prevent edema after
the sprain.”
10 – Urinary Catheterization
“The doctor will order urinary catheterization for male patient to relieve urinary retention
by introducing a catheter through the urethra into the bladder to remove fluids and will
promote comfort to the patient and prevent risks of illnesses caused by distention.”
11 – Oxygen therapy
“The doctor will order the administration of oxygen therapy to the patient to provide
enough oxygen to the lungs of the patient to keep the organs healthy and prevent the
manifestation of hypoxia.”
12 – Cleasing Bed Bath
“I am expected to do a cleansing bed bath for the patient to promote comfort, maintain
hygienic environment, reduce the transmission of microorganisms, and prevent the
manifestation of any illnesses caused by being soiled and dirty.”
13 - Tepid Sponge Bath (42 degree c)
“My nursing action would be to do a tepid sponge bath. This is help to reduce the fever
of the patient and also to replenish the patient’s buccal membrane by hydrating the
patient if not contraindicated, this will also help to lower the body temperature.
14 – Occupied bed
“I will perform occupied bed to maintain a hygienic environment, provide comfort for the
patient, prevent manifestation of microorganisms, and to conserve the patient’s energy
since she has her left leg cast on.”
15 – Blood transfusion
“Since the admitting physician ordered to have it replenish, we have to do a blood
transfusion. This will restore the circulating blood volume after the trauma of the lower
left extremity. This will also prevent severe anemia and blood loss that will possibly lead
to death.”
16 – Warm Compress
“We will apply a warm compress to increase the circulation, boosts the of blood and
nutrients, and provide comfort.”
