MEMORANDUM OF AGREEMENT (MOA)

TO FACILITATE MEDICAL SERVICES LIFE COACHING, STRESS DEBRIEFING,

COMFORTING AND PROCESSING TO HELP THE VICTIMS AFTER THE OCURRENCE
OF DISASTERS, CALAMITIES OR EMERGENCIES IN THE MUNICIPALITY OF DOÑA
REMEDIOS TRINIDAD, BULACAN”

KNOWN ALL MEN BY THESE PRESENTS:

This Agreement made and executed into this 1st day of December 2022 by and between:
The Municipality of Doña Remedios Trinidad, a local government unit (LGU) in the
Province of Bulacan, Region III, duly established under the laws of the Republic of the Philippines,
with address at Poblacion, Pulong Sampaloc, Doña Remedios Trinidad, Bulacan herein represented
by its Municipal Mayor, HON. RONALDO T. FLORES, herein after referred to as the First Party;
-and-
The Sto. Niño Hospital with address at Poblacion, Bustos, Bulacan represented herein by
PABLITO MENDOZA, JR., MD herein referred to as the Second Party;

WITNESSETH:

WHEREAS, R.A. 10121 also known as the Philippines Disaster Risk Reduction and
Management Act of 2010 provides for the development of policies and plans and the implementation
of actions and measures pertaining to all aspects of disaster risk reduction and management,
including good governance, rick assessment and early warning, knowledge building and awareness
raising, reducing underlying risk factors, and preparedness for effective response and early recovery.

WHEREAS, the local government units are mandated to uplift the lives of its constituents
and as a whole to be progressive, competitive and a safe community;

WHEREAS, business such as store, supermarkets, and pharmaceutical companies, medical

groups or and/or psycho-social professionals from the government or NGOs are operating under the
existing laws to provide the need for food, medical supplies and services and commodities needed by
the general public for a fee or other considerations;

WHEREAS, medical groups or and/or psycho-social professionals from the government,

NGOs and private organizations provides medical services which are vital after the occurrence of
disasters, calamities or emergencies to help the victims to return to their normal way of life free from
emotional/ mental torture cause by disasters, calamities or emergencies.

NOW, THEREFORE, for and in consideration of the foregoing, the parties here to agree
with the content of this agreement:
 A. ON THE PART OF THE FIRST PARTY:

1. Allocate/Appropriate adequate funds to cover the cost or pay the amount of medical
services requested to the second party subject to existing government, laws, rules and
regulations;

2. Provide assistance of effective mechanism for the smooth flow of this undertaking;

3. Monitor/evaluate the effectiveness of the systems and processes used on this undertaking.

B. ON THE PART OF THESECOND PARTY

1. Provide the following services:

 Medical services, life coaching, stress debriefing, comforting and processing

after the occurrence of any disasters, calamities and emergencies to the
victims.

2. Perform the above-mentioned activities to the place or area as determined by the First-
Party upon the presentation of approved request;

3. Collect the amount due for services rendered subject to existing pertinent government
laws on budgeting, accounting and auditing rules and regulations

The Agreement shall commence immediately upon signing of the MOA and shall remain in
full force and effect unless sooner mutually terminated by the parties concerned

Done in the Municipality of Doña Remedios Trinidad, Bulacan, Philippines, this 1st
day of December 2022.
