I, JAY-R P. MALAYAG, of legal age, Single, Filipino, and a resident of 105 Apitong St.
Balite Lagao, General Santos City, South Cotabato, after having been duly sworn to, in
accordance with law, do hereby depose and say:

1.) That I have been in government service since January 07, 2019;

2.) That my employment from the city government shall sever effective October 30, 2021;

3.) That during my employment with the government, specifically, the Local Government
of General Santos City, no administrative charge of any offense in any court,
administrative or quasi-judicial bodies was filed against me;

4.) THAT UPON MY END OF TERM/RETIREMENT/RESIGNATION, THERE IS NO

PENDING CRIMINAL INVESTIGATION OR PROSECUTION FILED AGAINST ME;

5.) THAT UPON CLAIM OF MY TERMINAL LEAVE BENEFIT, I AUTHORIZE THE CITY
GOVERNMENT TO DEDUCT ALL MY FINANCIAL OBLIGATIONS WITH IT;

6.) That I execute this affidavit to attest to the truth of the foregoing and to support my
claim for terminal leave benefits.
