I. RATIONALE
The Universal Health Care Act (UHC) of 2019, or the Republic Act 11223, states that every
Filipino shall be granted immediate eligibility and access to care through a registered network of
health care providers known as the Health Care Provider Network (HCPN). To ensure that health
infrastructure inputs are available and appropriate to deliver the necessary health services following
the HCPN framework’, the Department of Health (DOH) developed the Philippine Health Facility
Development Plan (PHFDP) 2020-2040 which articulates the required investments for responsive,
equitable, and resilient health facilities.

To monitor the implementation of the PHFDP including the supply of health facilities and
equipment and the quality of health services, the Health Facility Profiling (HFP) System’ was
established in 2021 for hospitals and infirmaries across all regions through Department Memoranda
2021-0419° and 2021-0443‘. The HFP System was embedded the DOH Data Collect Hub Website
(hfpddc.doh.gov.ph) which enables the identification of gaps in health facilities towards the
in
system-wide strengthening of health facility networks. The HFP system also aims to integrate and
simplify data collection from hospitals and infirmaries related to the Integrated Hospital Operation
Management Program (IHOMP)° and Department Circular No. 2022-0293°.

In this light, the DOH hereby issues these guidelines to start the conduct of the Health
Facility Profiling for 2022 in all regions. The annual data generated from this will also be used to
re-launch the Health Atlas Website of the Knowledge Management and Information Technology
Service (KMITS), which is a public-facing website with information on the geographic distribution
of facilities and their service capability.

Il. OBJECTIVE

This issuance aims to provide guidance on the conduct of the annual Health Facility Profiling
among all government and private hospitals, infirmaries, and primary care facilities.

Il. SCOPE OF APPLICATION

These guidelines shall cover all government and private hospitals, infirmaries, and the
respective DOH Centers for Health Development (CHD) in all regions in the Philippines and
Ministry of Health-Bangsamoro Autonomous Region in Muslim Mindanao (MOH-BARMM).
IV. IMPLEMENTING GUIDELINES

All primary care facilities, hospitals, and infirmaries are directed to submit reports to the
Health Facility Profiling System (hfpdde.doh.gov.ph). Questionnaires for all the modules will be
uploaded in the HFP Files Google Drive (bit.ly/HFP-AVDrive22).
A. Primary Care Facilities

1. All government primary care facilities (PCF) shall accomplish the Primary Care Facility
module by September 30, 2022 (deadline) through the HFP System found in the DOH
Data Collect Hub (hfpddc.doh.gov.ph).

2. For private PCFs, the CHDs and MOH-BARMM will first need to register these facilities
in their respective regions in the National Health Facility Registry. Afterwhich, private
PCFs will then be given HFP accounts which will be used in accomplishing the Primary
Care Facility module by October 30, 2022 (deadline) through the HFP System found in
the DOH Data Collect Hub (hfpddc.doh.gov.ph).

3. The online orientation on how to navigate through the application and how to answer the
PCF module is scheduled on August 9-10 (Tuesday and Wednesday) through Webex.
Kindly refer to the schedule and batches of online meetings below via Webex:
a. Batch 1: PCF Online Orientation (August 9, 9:00 AM to 11:00 AM)
b. Batch 2: PCF Online Orientation (August 9, 1:30 PM to 3:30 PM)
c. Batch 3: PCF Online Orientation (August 10, 9:00 AM to 11:00 AM)
d. Batch 4: PCF Online Orientation (August 10, 1:30 PM to 3:30 PM)

4. Registration for the Webex online orientation is required before entering the meeting
room. The link for the registration for the PCF Profiling orientation is

5. After the orientation, audiovisual recordings will be readily available through the PCF
HFP Files Google Drive (https://bit.ly/PCFHFPDrive22).

6. All PCFs are directed to submit reports starting August 15, 2022.

B. Hospitals and Infirmaries

1. All hospitals and infirmaries shall accomplish the following modules by October 30,
2022 (deadline):
Hospital Profile;
Nursing Module;
Pharmacy Module;
Quality Module;

Infection Prevention and Control Module;


Public Health Unit Module; and
Medical Certification of Cause of Death Module.

2. The online orientation on how to navigate through the application and how to answer
each module is scheduled on August 2, 2022 (Tuesday) through Webex. Kindly refer to

Cause of Death (MCCOD) Assessment Module

5. The MCCOD Assessment Module in the HFP uses a separate log-in credential/account
for each hospital.

6. All hospitals and infirmaries are directed to submit reports starting August 5, 2022.

C. Centers for Health Development (CHDs) and Ministry of Health-Bangsamoro


Autonomous Region in Muslim Mindanao (MOH-BARMM)
1. All CHDs and MOH-BARMM shall register and update all government and private
primary care facilities in the National Health Facility Registry (NHFR). The following
offices are responsible for the following specific health facilities:
a. City/Provincial Department of Health Office (C/PDOHO) for private medical
outpatient clinics, Rural Health Units (RHUs), and Urban Health Centers (HCs); and
b. Regulations, Licensing and Enforcement Division for licensed private PCFs and
Ambulatory Surgical Clinics.
Moreover, the outsourced publicly available list of private medical outpatient clinics and
ambulatory surgical clinics from different websites were pooled in Google Drive
(https://bit.ly/PrivateClinics2022) for CHDs’ reference in the NHFR registration
process of private PCFs within their respective region.
2. All CHDs and MOH-BARMM through the Health Facility Development Unit (HFDU)
shall supervise and ensure the submission of all government and private primary care
facilities, infirmaries, and hospitals in the Health Facility Profiling System.

3. All RLED, C/PDOHO, HFDUs, and other concerned CHD and MOH-BARMM staff
are
invited to an online orientation for the 2022 Health Profiling System on July 28, 2022
(Thursday), from 9:00AM to 12:00NN via Webex.
4. Registration for the Webex online orientation is required before entering the meeting
room. The link for the registration for the CHD and MOH-BARMM orientation is

V. ROLES AND RESPONSIBILITIES

The listed concerned offices shall perform the following with regard to the Health Facility
Profiling System:
A. All Public and Private Primary Care Facilities shall:
1. Update its account details and log in the HFP system through the DOH DataCollect
Website;
2. Comply with 2022 annual submission of data through the HFP System within the given
timeline; and
3. Ensure that the submitted information through the HFP is vetted by the head of the
facility or personnel authorized by the management.

B. All Chiefs of Public and Private Hospitals and Infirmaries shall:


1. Update its account details and log in the HFP system through the DOH DataCollect
Website;
2. Comply with 2022 annual submission of data through the HFP System within the given
timeline; and
3. Ensure that the submitted information through the HFP
is
vetted by the Medical Center
Chiefs, Hospital Chiefs, or personnel authorized by the hospital management.

C. The Centers for Health Development and Ministry of Health-Bangsamoro Autonomous


Region in Muslim Mindanao through their respective Health Facility Development Units
shall:
1. Properly disseminate information and communicate (i.e. advisories) with primary care
facilities, hospitals and infirmaries regarding the online platform;
2. Monitor submissions of health facilities including the response of rate of health facilities;
3. Provide technical assistance to health facilities regarding frequently asked questions
4. Conduct quality control to ensure accurate submission of data among health facilities; and
5. Submits annual report on MCCOD Assessment to DOH-HFDB
per DC No. 2022-0293,
Interim Guidelines on the Assessment of Medical Certification of Cause of Death
(MCCOD) in Health Facilities.

D. The Centers for Health Development and Ministry of Health-Bangsamoro Autonomous


Region through the following office/division shall register and update all government and
private primary care facilities in the National Health Facility Registry (NHFR):
1. Provincial Department of Health Office (PDOHO) for private medical outpatient clinics,
Rural Health Units (RHUs), and Urban Health Centers (HCs); and
2. Regulations, Licensing and Enforcement Division for licensed private PCFs and
Ambulatory Surgical Clinics.

E. Health Facility Development Bureau (HFDB) of


the DOH shall:
1. Develop tools with validation formulas that will be used in collecting data in the HFP
system;
2. Orient HFDUs and primary health care centers and hospitals in the implementation of the
HFP system;
3. Coordinate with HFDUs regarding the monitoring of data submissions; and
4. Analyze data and produce final reports based on the data collected every year.

F. Knowledge Management and Information Technology Service (KMITS)


1. Be in charge of the application/website development of the HFP system;
of
the DOH shall:

2. Schedule pre-test of HFP system website and preliminary visualizations for quality
control; and
3. Provide simple software technical assistance to hospitals and infirmaries regarding the
use of the HFP system website.

VI. REPEALING CLAUSE

The roles and responsibilities of CHD divisions and units regarding adding and updating of
health facilities and offices in the National Health Facility Registry based on DM 2022-0056,
“Updating of Health Facilities and Health Offices in the National Health Facility Registry (NHFR)”,
are hereby amended.
