LETTER OF CONSENT

conduct of the mixed-method study entitled "Causes Provoking Adolescents' Engagement
Toward Premarital Sexual Intercourse." I understand that the purpose of this study is to
explore the factors that contribute to adolescents' engagement in premarital sexual
intercourse.
I acknowledge that my participation in this study is voluntary and that I am free to withdraw
from the study at any time without any negative consequences. I understand that my
participation in this study will involve the completion of a survey and a possible interview
with the researcher.
I understand that my responses will be kept confidential and that my anonymity will be
maintained. I understand that the data collected from this study will be used solely for
research purposes, and will be reported in an aggregate form without any identifying
information.
I understand that the results of this study may contribute to a better understanding of the
factors that contribute to adolescent sexual behavior, which may inform the development
of effective interventions to promote healthy sexual behaviors.
