METHODOLOGY FOR FORM WORK

SECTION-01: PURPOSE
The purpose of the document is to describe the work procedure to be
adopted for Formwork in construction site.

SECTION-02: SCOPE
This procedure applies to the installation of formwork and the controls
required during assembly, erection and final positioning of formwork used in
Structural concrete operations.

SECTION-04: RESPONSIBILTIES

Following are the responsibilities and reporting structure of personnel


involved in the scope of the procedure.

Project Manager: - In charge of all activities at site.

Site Incharge:
- Shall be report to project manager
-Shall be responsible for supervision, control, Implementation and execution
of plans
-Shall also be responsible for supervision of construction activities and
preparation of construction records Develop construction schedules,
procedures and prepare progress and accomplishment reports.

Site Engineers:
-Shall report to Site in charge.
-Shall be responsible for day to day planning of site, Construction activities
and preparation of construction records.
Survey Engineer/Surveyor:
-Shall report to Project Manager

QA/QC Engineer:
-Shall report to Project Manager
-Shall be responsible for overall control of QC
-Supervision of activities and sub-contractors QC activities, implementation
of Quality plan.

Safety Engineer:
-Shall report to Project Manager
-Shall be responsible for overall control of safety system.
-Supervision of activities and sub-contractors
-Safety activities, implementation of safety plan.

METHODOLOGY FOR FORM WORK:


1    All formwork activities will be carried out in accordance with the
specification.

.2    After studying the structural drawings, the Project Engineer will plan
for sizes, numbers and type of formwork to be used in consultation with the
Structural Engineer, if required. Depending upon the programme, the
number of repetitions will be decided.

Availability of formwork will be checked with the Procurement Manager and


decision will be taken to buy, hire or reuse formwork material.

.4    During fabrication of formwork shutters, all dimensions shall be cross-


checked with a view to avoiding corrective action during erection.

.5    Paint reference numbers on all panels to ensure their use in correct


positions.

.6    Ensure that the props, shores, waling, bearers, clamps and tie rods are
the right size and at the correct spacings. The Formwork designer shall
check the falsework system with respect to load imposed on it and design
parameters as laid down in the specification.

.7    Check that the falsework is securely braced and is on a firm


foundation.

.8    Forms fastened to previously cast concrete must be tightly fixed to


prevent grout loss. Cellular foam plastic strips can be used to make a seal.

.9    Check quality of shutter lining, tightness of bolts and wedges, built-in


items, inserts and other embedment’s.

.10    Tie – rod holes to be made in the formwork shall be neat so that they
can be patched or plugged later on.

.11    Particular attention will be paid to the rigidity and line of stop ends
and joint formers.

.12   Remove all tie-wire clippings and nails, which may stain both the
formwork and the concrete.
.13    Ensure that adequate access and working platforms are in place for
the concreting gang and that toe boards and guardrails are provided.

.14    A spreader or lifting beam shall be used to prevent distortion when


placing formwork.

.15    Adjustable steel props shall be erected in plumb and verticality of


props checked using a spirit level.

.16    A prop should not be used if it is bent, creased or, rusted or has a
bent head or base plate, damaged pin etc.

.17    Proper release of agent shall be used so that it can be removed


without damaging the surface finish of the concrete.

.18 New timber and plywood forms shall be given first coat of the
appropriate release agent 36 hours in advance, then a second coat just
before they are used. Avoid excessive use of release agent as this can cause
staining or retardation of the concrete.

.19   A close watch should be kept on all wedges and fastenings. Make sure
that they do not work loose. Grout loss is an indication that some movement
has occurred.

.20 When unsleeved tie bars are being used, ease them slightly before the
concrete sets, and remove them as soon as it is safe.

.21 Striking the Formwork:

i)    Always obtain approval before striking the formwork in the form of


Appendix CONC/04/A.

ii)    Ensure adequate safety measures while striking formwork.

iii)    Cleaning the face of forms shall start as soon as they have been struck.

iv)    Good storage is essential to ensure that formwork can be used again.

v)    Paint code numbers on them so that they can be readily identified when
next needed.
SECTION-6 : HEALTH, SAFETY AND ENVIRONMENT

i) The weights of large prefabricated sections of formwork should be marked


on them so that the lifting capacity of the crane at the working radius is not
exceeded. Lifting points are usually provided.

ii)    If necessary, a spreader or lifting beam shall be used to prevent


distortion.

iii)    Where lifting is done by the vertical soldiers, check that they are
adequately connected to the walings.

iv)    Fire extinguishers in working order shall be in close proximity to the


stored formwork.

v) Skilled manpower with minimum experience in work is needed.

vi) No workers to be allowed to work at maximum heights with out safety


harness and nearby life line.

vii) Lighting should be provided in dark hours near the work in site.

viii) Materials used in Form work to be maintained good and no damaged


ones to be used in site.

ix) Dedicated supervisor and engineer to be deployed for the following works
carried out in the site.

SECTION-7 : QUALITY ASSURANCE AND QUALITY CONTROL

Quality control and assurance shall be strictly followed as per the approved
quality assurance plan and work method statement.

 The formwork and the bracket must have sufficient strength, rigidity
and stability. When installing and pouring concrete in concrete forms,
the formwork and its supports should be observed and maintained.
When an abnormal situation occurs, it shall be handled in time
according to the construction technical plan.

 When installing the upper formwork of the cast-in-place structure and


its support, the lower floor should have the carrying capacity to carry
the upper load or erect the support. The columns of the upper and
lower supports should be aligned and the pads should be laid.
 The joint of the formwork panels should not be fiber. Before the
shallow concrete, the wooden mold should be moistened with shallow
water, but the formwork does not bribe.

 The contact surface between the formwork and the concrete should be
smooth and painted with a release agent, but no release agent that
affects the structural properties or hinders the construction of the
decorative works.

 The formwork and support must have sufficient strength, rigidity,


stability and precision to ensure the geometrical dimensions and
elevation of the components. The surface of the formwork is flat and
the seams are tight.

 Pre-construction formwork, support detailing and construction steps


(including demoulding) shall be reviewed and signed by the project
technical person in charge. The key parts, especially the support of
large space and large span parts, shall be checked and reported to the
company’s chief designer for approval. It can be implemented at the
rear to ensure its safety.

 The floor structure formwork and the removal of the support shall be
demolished after the floor has reached the specified strength. The
removal of the special formwork shall be subject to the approval of the
project technical person in charge and the supervision of Party A.

 In the construction of concrete structures, where the joints of


columns, walls and masonry are required according to the design
requirements, in the vertical direction of the wall, anchored into the
column and wall is not less than 700mm, and the outside of the
column stirrups is tied. 6 steel bars, the upper and lower spacing is
200mm for demoulding. The masonry is welded before construction,
and the welding length is implemented according to the requirements
of single-sided welding.

 Reinforced concrete columns/pedestals connected to the ring and the


beam, the wall shall be placed in the circle and the longitudinal
reinforcement of the beam shall be ribbed, and the anchored into the
column shall not be less than 35d, and shall protrude from the wall
and be not less than 700mm outside the column.

 The acceptance of the formwork project focuses on the control of


stiffness, verticality and flatness, paying particular attention to the
correct position of the template axis at the peripheral formwork,
column formwork, elevator shaft, stairwell, etc.

 Before installing the column formwork, a steel bar with a diameter of


not less thanΦ10 must be added at its root (concrete guide wall is
strictly prohibited) to ensure its correct position. The lower part of the
formwork is leveled with 1:3 cement mortar to ensure the uniformity
of the formwork elevation.

 The debris in the formwork before pouring concrete should be cleaned


up.

 For embedded parts fixed on the formwork, the reserved holes shall
not leak slurry, and the annual solid shall be installed, and the
deviation shall comply with the regulations.
