HEART FAILURE QUESTIONNAIRES

TRUE OR FALSE:
1. Heart failure is a disease.
Correct Answer: False
Heart Failure is not a disease, it is a syndrome characterized by manifestations of volume of
overload, inadequate tissue perfusion, and poor exercise tolerance.

2. Elevating the head of bed may help the client with dyspnea.
Correct Answer: True
Elevating the head of bed such as high-fowlers position promotes widening of airway, avoids
chest compressions, and reduces pulmonary congestion.

3. When administering diuretic mediations the nursing consideration is to monitor client's weight,
intake, output, and serum electrolyte levels.
Correct Answer: True
Monitoring helps to determine the progress of the treatment and also to assess for any changes
in the client’s status. After administering diuretics, monitor the potassium level; watch out for
decreased potassium levels (hypokalemia).

4. Restriction sodium intake and limitation of water intake for a 1L/day applies to the client who
have edema.
Correct Answer: True
Too much intake salts and fluids may accumulate more fluids in the body which causes thirst
and swelling.

5. Only men can develop a HF?

Correct Answer: False
Both men and women can have HF at any age, but it is common to older 65 years old.

MULTIPLE-CHOICE:
6. What test is a type of ultrasound scan that is use to assess the structures and functions of the
heart.
a. Echocardiogram
b. Echocardiagram
c. Electrocardiagram
d. Electrocardiogram
Correct Answer: A
Echocardiogram is a non-invasive procedure and a type of ultrasound scan which shows and
assess the structures and function of the heart.

7. What are the common causes of HF? SATA

a. Cardiomyopathy
b. Heart Attack
c. Coronary Artery Disease
d. None of the above

Correct Answer: A, B, C
The common causes of heart failure are coronary artery disease, cardiomyopahty, and heart
attack.
Coronary Artery Disease (CAD) is a problem with the arteries that carries and supply blood and
oxygen to your heart. CAD is most common cause of heart failures.
Heart Attack may happen when a coronary artery is blocked suddenly, which stops the flow of
blood to your heart muscle.
Cadiomyopathy refers to damage to heart muscle that can make it harder to heart to pump
blood.

8. What side of the heart fails when you hear bibasilar crackles upon auscultation of lungs?
a. Left-sided HF
b. Right-sided HF
c. Upper-sided HF
d. None of the above
Correct Answer: A
Pulmonary congestion or respiratory distress is most common manifestation in left-sided HF.

9. It is the most common cause of heart failures.

a. Hypertension
b. Coronary Artery Disease
c. Heart Attack
d. Congenital Heart Disease
Correct Answer: B
• Coronary Artery Disease (CAD) is a problem with the arteries that carries and supply
blood and oxygen to your heart.

10. It is the most common diuretic medication used for Heart Failure.
a. Metolazone
b. Thiazide
c. Chlorothiazide
d. Flurosemide
Correct Answer: D
Furosemide is a loop diuretic medication used to treat fluid build-up due to heart failure, liver
scarring, or kidney disease. It may also be used for the treatment of high blood pressure.

11. It is used to relieve hypoxia and dyspnea and to improve oxygen-carbon dioxide exchange.
a. CPAP
b. BIPAP
c. Supplemental Oxygen
d. High-Fowler's position
Correct Answer: C
Supplemental Oxygen is a teatment in which a storage tank of oxygen or a machine called a
compressor is used to give oxygen to people with breathing problems.

12. It helps to correct an irregular or slow heartbeat.

a. MCSD
b. Pacemaker
c. VADs
d. Heart Transplant
Correct Answer: B
Pacemaker is an artificial device for stimulating the heart muscle and regulating its contractions.

13. It is a test that measures the levels of oxygen and carbon dioxide your blood.
a. Complete Blood Count (CBC)
b. Brain Natriuretic Peptide (BNP) Blood Test
c. Arterial Blood Gas (ABG) Test
d. Liver Function Test
Correct Answer: C
An arterial blood gas (ABG) test is a blood test that requires a sample from an artery in your
body to measure the levels of oxygen and carbon dioxide in your blood.

IDENTIFICATION:
14-15. Give the 2 types of left-ventricular heart failure:
Correct Answers: Heart failure with reduced ejection fraction (HFrEF) or Systolic failure
and Heart failure with preserved ejection fraction (HFpEF) or Diastolic failure
The 2 types of LVHF are; Heart failure with reduced ejection fraction (HFrEF) or Systolic failure
and Heart failure with preserved ejection fraction (HFpEF) or Diastolic failure.
Heart failure with reduced ejection fraction (HFrEF) or Systolic failure happens when the left
ventricle loses its ability to contract normally. The heart can't pump with enough force to push
enough blood into circulation.
Heart failure with preserved ejection fraction (HFpEF), also called diastolic failure happens
when the left ventricle loses its ability to relax normally (because the muscle has become stiff).
The heart can't properly fill with blood during the resting period between each beat.

16-20. Give the 5 nonpharmacological management of Heart Failure.

• ABC’s – Assess airway, breathing, and circulation in the initial evaluation and initiate
management based on the status of the patient.
• Continuous Positive Airway Pressure (CPAP) and Bilevel Positive Airway Pressure
(BIPAP) are non-invasive methods to support respiratory that treats respiratory insufficiency
secondary to pulmonary vascular congestion and pulmonary edema. If CPAP and BIPAP are
not effective to improve the patient’s respiratory status, an early intubation and mechanical
ventilation are considered to prevent further decompensation and progression of symptoms.
• Dietary and Lifestyle Changes (i.e., decrease salt intake, limit/restrict water intake to
1000mL/day (1L/day) especially if edema is present, exercise regularly, smoking cessation,
stop drinking alcohol).
• Elevate Client’s Head the client should be placed in a high fowler position to reduce
pulmonary venous congestion and to relieve dyspnea.
• Supplement Oxygen is used to relieve hypoxia and dyspnea and to improve oxygen-carbon
dioxide exchange.
