School-Wide Instructional Technology Coaching Program Evaluation Instrument

Plan process:
1. Teachers will attend technology sessions at least once a month
2. At the end of the professional learning sessions will be a participant questionnaire
3. After implementation, teachers will use the provided checklist to describe how the technology has impacted them and their
students
4. Teachers will provide a lesson plan that shows how they implemented the technology resource
5. Teachers can volunteer to demonstrate the implementation of the technology
