Policies for Partial Withdrawal / Dropping / Changing / Adding of Subjects / Shifting Course
Changing may be allowed only once:
Before action shall be taken on the student’s request for changing/dropping or adding of subject(s) or change
of course, the student:
a) Shall attach to this form his/her official class schedule
b) Shall manifest an assurance that he/she has fully understood and has complied with the existing policies
on prerequisites and term load limitations. Any misinterpretation or misstatement committed by the
student shall constitute a sufficient ground for disciplinary action to be taken against him/her.
Only the College Dean or his duly designated representative may approve the request for changing/adding,
dropping of subject(s) or change of schedule or course.
When approved, present this form to the Computerized Enrolment Room or the University Registrar’s Office for
the entry and printing of the Final Student Schedule.
Note:
Class codes for subjects to be added or changed to will be determined at the Computerized Enrolment
Room depending on the availability of class codes.
Withdrawal, Adding, Changing or Shifting Course is considered official upon signing of the personnel
from the Computerized Enrolment Room/ University Registrar’s Office.
