Checklist of requirements for SSS:

1. Two (2) copies of the form without erasure and alterations

2. Fill out the Tax Identification Number
3. Must be signed by the president, chairman or corporate secretary
4. Form must be notarized.

Documents to be submitted:

Any of the following documents: (Must be original and photocopy)

1. Certificate of Registration of Business Name from department of Trade and Industry

2. Certificate of Filing of Articles or Amended AOI from SEC

For Philhealth:

1. Form must be in duplicate copies

Requirements:

1. Amended AOI duly received by SEC

For Pag-ibig:

Requirements:

1. Employer’s Change of Information Form (One Original)

2. Valid ID (1 Photocopy)
3. Specimen Signature Form (One Original)

Original documents must be presentted for authentication.
