Conversation Script (Taking patient Observation)

Example 1

Nurse : Good morning, …….(sebutkan nama pasien anda)

Patient : Good Morning Nurse.

Nurse : How are you doing today?

Patient : a bit better thank you.

Nurse : Ok, ……(sebutkan nama pasien anda). Now I am going to take your obs.

Patient : Sorry?

Nurse : it is an observation. For now, I need to check your Blood pressure, temperature, oxygen

saturation, and also your weight.

Patient : Ohh, Ok.

Nurse : First, I will check your Blood pressure using this digital blood pressure. Could you put your

arm out straight, please.

Patient : Ok.

(perawat mencatat hasilnya)

Nurse : Now I will check your………………using…………..(beri instruksinya, seperti Tindakan diatas)

(Setelah selesai di check semua. Kemudian disampaikan hasilnya)

Contoh:

Nurse : Ok,…… (sebutkan nama pasien anda). The obs is done. All is good but your blood pressure

is little bit high. It is…(sebutkan hasilnya)…

Patient : Thank you Nurse

Example 2

Nurse : Excuse me, ………(sebutkan nama pasien anda), right?

Patient : Yes, Nurse

Nurse : My name is………I’m looking after you today.

Patient : Thanks nurse.

Nurse : for now, I am going to take your observation.

Patient : what is that?

Nurse : I am going to check your (Sebutkan apa saja yang akan diperiksa)

Patient : oh, Ok.


Nurse: Well, ……(sebutkan nama pasien anda) now, I will check your temperature first. Could you
open your mouth, please. Hold out till the bip. That is the bip. Mr/ Mrs…….your temperature is quite
high. It is 48,2.

Nurse: …….(lakukan seperti yang diatasnya)

Nurse:……. (lakukan seperti yang diatasnya)

Setelah selesai semua mengobservasi kondisi pasien dan menyebutkan hasilnya untuk penutup

Nurse: Ok, Mr/ Mrs……the obs is done. you can continue resting

Patient: Thank you nurse.
