Your Member ID Card

You and each member of your household covered by Health First Colorado receive a member ID
card. If you have an old card, please get rid of the old one to avoid confusion.

This card has everything you need

We want it to be easy for you to get the help you need. Take a minute to read your card. Your card
tells you who to call for appointments, emergencies, your coverage status, help with care, dental,
benefits and billing, and transportation. If there are any mistakes, please call 719-636-0000 to get a
new card.

Get your card on your phone with the Health First Colorado app
Use your PEAK account login to get your card on the free Health First Colorado mobile app.
Download it from the app store. The Health First Colorado app shows your current coverage status
and will have the most up-to-date information on your member card.

Carry your card

You can also use the Health First Colorado app and never lose your member card again. Or take
your paper card with you to your doctor or pharmacy. You can use the space on the back of your
card to write important information about you.
