KNOW ALL MEN BY THESE PRESENTS:

It is this ________ day of____________, 2___ in the city of____________, mutually

agreed between the ________________SS Co., OWNER of the good ship ___________ of
___________________ tons net register or thereabouts, now lying at the port of_____________,
and ___________________________ CHARTERER:

That the said ship being tight, staunch, and strong, and in every way fitted for the voyage,
shall with all reasonable dispatch proceed to________________ and there load full and
complete cargo of _______________ and/or other lawful merchandise, and, being so
loaded shall proceed to________________, or so near thereto, as she may safely get, and deliver
the same, on payment of freight, at the rate of ____________ per bale of ____________kilos,
with liberty to call at any port or ports on the way (the Act of God, the King’s enemies, restraints
of princes, fire, and every other peril of the sea always mutually excepted.)

Ten days to be allowed for loading and discharge and five days on demurrage over and
above the said lay days at P_________________ per working day.
