dead body or remains or cremation of dead body of

Vietnamese overseas to Vietnam

1. In order to apply for a Permit to return dead body or remains or cremation of dead
body of Vietnamese overseas to Vietnam, the concerned agencies or individuals
are kindly requested to submit a dossier including the followings:

- Application Form (n xin a thi hi/di hi v nc) filled out by the
applicant (on the following page);
- Death Certificate;
- Quarantine Control paper issued locally cerifying that no infectious disease
is found and the remains or cremation is permitted to be transported
overseas.;
- Burial permit issued by the authorities in Vietnam (local Peopless
Committee) where the remains is to be buried (disregards in case of
transporting cremation/ashes).

2. The dead body/remain must be placed in a hermetically sealed metal casket. The
ash of cremated dead body must be contained in a hermetically sealed urn.(Please
consult the transporting airline company for specific requirements).
3. The fee should be paid by company check or money order, payable to the
Consulate General of Vietnam. Please contact the consular section of the
Vietnamese Consulate General in San Francisco for the present fee list.
4. If the processed dossier with the Permit is expected to be returned by mail, a
prepaid self-addressed envelop (Federal Express mail is advised) has to be
attached with the mail-in dossier.
5. The Consulate General of Vietnam shall process the dossier and issue the Permit
in one day after the complete set of papers mentioned above and the fee are
received.
