I agree that FedEx has the absolute discretion to decline my application without giving any reason and retain
documents submitted as property of FedEx, and that FedEx reserve its right to suspend my account for reasons
wholly within FedEx discretion including but not limited to misuse, abuse and non-settlement of payments.

I agree to indemnify FedEx and its affiliates for all costs, expenses or loss in connection due and payable to FedEx or
its affiliates.

I understand that FedEx will collect and process personal information or sensitive personal information in relation to
providing me its services. FedEx reserves the right to dispose of such information in accordance with applicable laws,

Using your personal information
1. Federal Express Pacific Inc. (“we” or “us”) collects personal information or sensitive personal
information (“personal information”) when you register with us or place an order for products or
services.

2. We will use the personal information you provide to provide the products/services, and to
process your payment for such products/services.

3. In processing your order or processing your payment, we may share your personal

9. By completing this form, you hereby authorize FedEx Express, its parent and affiliates (collectively
“FedEx”), and their authorized agents, service providers and business/marketing partners to use the
aforementioned information and apply on your behalf for: (1) a login user ID and default password of the
FedEx official website (fedex.com/ph) (2) FedEx® Billing Online. You further agree that upon receipt of
the user ID and default password from FedEx Express, you will log in the website and verify all data filled
out on its behalf. For the sake of data security, please immediately change the password by yourself. By
completing this form, you further consent to FedEx and its authorized agents, service providers and
business/marketing partners to collect, use, disclose, transfer and share this data (including across
jurisdictions) for relationship management, marketing, advertising and promotion purposes. These
purposes include any other purpose as authorized by you, or as otherwise required or permitted by law.
You also agree that: (a) the consent herein overrides any earlier withdrawal of your consent; (b) the
consent herein does not supersede or replace any other consents which you may have provided to
FedEx, their authorized agents, service providers and/or business/marketing partners; and (c) FedEx,
