1. Medical and Nursing History. 6 years ago, patient was diagnosed with type 2 diabetes. 2 years ago, patient was
diagnosed with a frozen shoulder and acquired COVID-19 virus.
2. Nutritional Assessment.
Anthropometric Measures. Height is 172cm. Weight is 176 lbs. BMI is 27.0 classified as pre-obesity based on the WHO BMI
Classification
Dietary History. Cabbage & Century Tuna. Time usually eats at 11:00 am, three times a day. Often food intake is vegetable.
The patient rarely drinks coffee but often drinks milk.
Eating/Feeding Habits/Practices. Eating bread and milk in the morning, vegetables and bread in the afternoon and
evening.

Modifiable Risk Factors. The patient doesn't eat rice but the food intake is considered a meal as advised by his doctor.
Previous use of vape and drinks alcohol occasionally.

6. Diagnostic Test Results. No available recent diagnostic test. The last diagnostic test was on July 2018 because of his
company’s protocol.

Medical and Nursing History. 8 years ago, patient underwent surgery for deep cuts on the skin. 4 years ago, patient
underwent abscess drainage.

Anthropometric Measures. Height is 152 cm. Weight is 119 lbs. BMI is 23.3 classified as normal weight based on the WHO
BMI Classification.
Dietary History. Fried-based food daily intake. Eating three times a day. Usually eats at 4:00 am, 12nn and the dinner
depends on what time the patient arrives home, latest is at 10:00 pm.
Eating/Feeding Habits/Practices. Eating three times a day and often eating chocolates after lunch or dinner.
