LEARNING CONTRACT
As a BUPCean student nurse, I commit myself to the following as I accomplish assigned tasks to
me:
1. To actively engage in the different alternative learning activities through distance
learning;
2. To personally answer all assessment tasks required for this course BY MYSELF ONLY;
3. To ensure academic honesty by keeping in confidence all my course requirements, I will
not copy from other students nor share my outputs to other students;
4. To safe keep my learning modules for my personal use and will not share the same nor
the password to my google class to others who are not enrolled in the class and may have
some self-serving interest;
5. To be careful and conscious not to plagiarize myself and others;
6. To give consent for the use/sharing of my course requirements for official academic and
administrative purposes;
7. To regularly communicate with my assigned faculty for updates, clarifications and timely
feedback; and
8. To continue to uphold the values of honesty and integrity.

As I take assessments/quizzes/examinations , I commit myself to the following:


1. I will personally answer the examinations given for this course by myself ONLY;
2. I will not record (screenshot or any form) the questions and answers and share this with
anyone who are also taking the same course.
3. I will not cheat in ANY FORM as I answer each items in this test.
4. If I have known of others who cheated in one way or another, I will immediately inform
my professor
5. I will be definitely HONEST in answering each item on this examination.
I understand that should I violate any of the above provisions, appropriate administrative
sanction may be imposed on me after due process.
