TO WHOM IT MAY CONCERN:

This is to certify that MR. CJ KLEIN BARIÑAN, 7 years old, male, a resident of Bitaug,
Enrique Villanueva, Siquijor was seen and examined by the undersigned today.

Clinical history revealed that patient is experiencing fever for 2 days associated with
vomiting and cough. Patient is advised for Complete Blood Count (CBC) with Platelet Count and
Urinalysis. Patient was also diagnosed with Acute Glumerulonephritis a month ago thus advised
for repeat urinalysis.

This certificate is issued upon the request of MS. SHERRY MAE BARIÑAN (mother) for
whatever legal purpose(s) this may serve best except for medico- legal purpose.
