ACCIDENT TYPES WHILE PERFORMING JHA:

1. Overexertion – lifting, pulling, pushing, bending, twisting and straining could be causes of
overexertion that could lead to fatigue.
2. Cumulative Trauma – awkward positions or repetitive movements, use of undue pressure or
force, use of tools which don’t fit the user or the job.
3. Stress – high output work involving production quotas, a great deal of mental concentration, or
other sources of outside pressure which cannot be relieved quickly or normally.
4. Caught In, Between or Under – moving equipment, in-running nip points, work underneath
moving equipment or materials
5. Struck by – falling, flying, or moving objects and equipment, work in open trenches or
excavations (cave-ins)
6. Struck Against – bump into or kick objects, be pushed or thrown down against something
7. Fall to Lower Level – Fall from equipment, stool, ladder, stairs, floor opening, etc.
8. Fall on Same Level – slip on liquids or grease, slip on ice, trip on obstacles on floors.
9. Rubbed or abraded – by pressure, vibration or friction.
10. Contact with Toxic Substances – caustics, acids, poisons, chemical irritants, etc.
11. Contact with Extreme Temperatures – environments which are too hot or cold.
12. Bodily Reaction – normal bodily motion or involuntary motion induced by sudden noise, fright or
loss of balance.
13. Contact with Electrical Current – use of power hand tools, electrical tools and machines in a wet
or moist environment, or working with electrical tools without protective insulation.
14. Hazardous Environments – work in confined space, in presence of flammable objects, and on
oxygen deficient atmospheres.

RECOMMENDED SAFE BEHAVIORS

- Never use tools or work equipment that you are not familiar with
- Always use proper lock-out and tag-out procedures to prevent unexpected movement or
operation of equipment being repaired.
- Shut off equipment before doing maintenance on that equipment.
- Always follow warning and hazard signs in the workplace.
- Don’t walk, run or drive equipment too fast in a stressful environment.
- Pass materials or tools to each other properly.
- Avoid jumping from elevations such as ladders, platforms, etc that might injure you.
- Never remove or inactivate safety guards, interlocks or other safety devices.
- Report damaged safety devices for immediate repair.
- Never use damaged or defective equipment.
- Always use proper tool for a specific job.
- Get assistance before trying to lift heavy loads that you cannot handle.
- Never operate an equipment without proper authorization or training.
- Always wear PPE required by the task being performed.

 Using tools or equipment without adequate knowledge.


 Fail to lock or secure materials and equipment against an unexpected motion
 Failure to shut off machine or equipment when it’s not necessary to use.
 Moving without precaution in congested areas or in confined places.
 Using defective equipment
 Using bare hands instead of hand tools provided for the job
 Overloading of trays shelves, carts, etc.
 Wearing defective, worn or unsuitable clothing for the task assigned.
 Failure to maintain systematic and periodic physical inspections of tools, equipment, and work
areas.
