1. Completed and signed application
2. Current transcript
3. A recent photograph; and
4. Two Scholarship Essay answers

Please submit all items as attachments in ONE email with your FULL name on each attachment

The DEADLINE to submit your application is Friday, May 12, 2023. Therefore, your application,
along with all of the items above, must be submitted via email no later than 11:59 P.M., May 12,
2023, to be considered. Any applications received with a later date will not be reviewed.

Scholarship winners will be notified on June 1, 2023.

All scholarship proceeds will be disbursed from the recipients’ financial accounts at the University of
Florida after the drop/add deadline in the Fall and Spring semesters.
