It is my pleasure to write this letter in support of Dr. Full Name of the Applicant’s (ECFMG/USMLE ID: ID) application to
the Exchange Visitor Sponsorship Program (EVSP) of the Educational Commission for Foreign Medical Graduates (ECFMG)
to change J-1 Visa Category from J-1 Research Scholar to J-1 Alien Physician. Dr. Name is an NRMP Match candidate for
our program starting on July 1, 2022.

Dr. Name is currently on a J-1 Research Scholar Visa at Name, Department, and Address of the institution. His/her
Research Trainee Visa expires on Expiry date on DS-2019.

This is to certify that Dr. Last Name will be a full-time employee of the Department of Medicine-Internal Medicine
Residency Program at the John. H. Stroger, Jr. Hospital of Cook County beginning July 1, 2022. Our categorical Internal
Medicine training program is of three years in length. Dr. Last Name will begin as a PGY-1 resident.

As per ACGME requirements, Research is a mandatory part of the internal medicine residency curriculum. Every resident
in our program is instructed to do a research project, participate in Journal Club, and present a lecture at the minimum.
Dr. Last Name’s previous research experience at the Name and Address of the institution - will help him practice
evidence-based medicine and patient care.
