Food allergies are a growing concern for both consumers and the food industry. Allergic reaction to foods can vary from minor to
life threatening. In an effort to help address this growing issue, we are requesting that you complete the following sheet for each
product that you supply Give and Go Prepared Foods.
Do you have effective procedures to avoid cross contamination of the product with the allergens not
present in the product but noted in columns 2 and 3?
If, for any reason, there are any modifications in this product, you are responsible for updating your
records and notifying us immediately.
