ADDENDUM TO ALTERNATIVE WORKFORCE

SOLUTION AGREEMENT

THIS ADDENDUM to the Alternative Workforce Solution Agreement, effective at the time it’s signed
by the Company, is by and between Solvo Solutions, LLC, a Delaware limited liability company ("Solvo")
and ______________________(the "Company").

RECITALS

A. Solvo and Company have entered into a legally binding Alternative Workforce Solution Agreement
("AWSA") under which Solvo has agreed to provide and administer services to Company.

B. This Pricing Addendum will determine the Services to be provided by Solvo and the Fees to be
paid by the Company.

PRICE AGREEMENT
Services shall be billed as a monthly flat rate (not hourly) for up to forty-five (45) hours a week (as identified
in the table below):

The value agreed between the parties should be understood as a unit value. Consequently, if the Company
requires additional services or additional personnel to be provided in addition to the employees initially
agreed, the same charges will be applied.

EFFECTIVE DATE
The Alternative Workforce Solutions Agreement and this addendum shall become effective at the time this
Addendum is signed by the parties.

IN WITNESS WHEREOF, the Parties acknowledge that each has carefully read this Addendum, that it has
been fully explained to them by counsel of their choice, that they fully understand its binding effect, that the
only promises made to them in signing this Addendum are those stated above, and that they are voluntarily
signing this Addendum. This Addendum is hereby entered into as of the date set forth below in the signature
block.
