ABSTRACT
The patient discharge process is a critical phase of the patient's care journey, as
it marks the transition from hospital to home. However, the discharge process
can be complex and challenging, leading to poor patient outcomes, including
readmissions, adverse events, and increased healthcare costs. Therefore, there
is a need to develop a patient discharge system that can improve the discharge
process and patient outcomes.

This project proposes to develop a patient discharge system that standardizes


the discharge process, improves communication between healthcare providers
and patients, and reduces the risk of adverse events and readmissions. The
project will use a mixed-methods approach to develop and evaluate the patient
discharge system, including a review of the literature, stakeholder interviews,
and a pilot implementation. The evaluation will assess the effectiveness of the
patient discharge system in improving patient outcomes, including reducing
readmissions and adverse events, as well as identifying barriers and facilitators
to the adoption and implementation of the patient discharge system.

The expected outcomes of this project are the development of a patient


discharge system that improves the discharge process and patient outcomes, as
well as identifying the factors that influence the implementation and adoption
of the patient discharge system. This project has the potential to reduce
healthcare costs, improve patient outcomes, and enhance healthcare efficiency
by standardizing and streamlining the discharge process.

1 CHAPTER 1: INTRODUCTION

1.1 Introduction:

The discharge process is a critical phase in

the patient's care journey, as it marks the

transition from hospital to home. However,

the discharge process is often plagued with

challenges, such as incomplete discharge

instructions, medication errors, and lack of

communication between

healthcare providers and patients. These

challenges can lead to poor patient

outcomes, including readmissions, adverse

events, and increased healthcare costs.

Therefore, there is a need to

develop a patient discharge system that can

improve the discharge process and patient

outcomes.

1.2 Problem Statement:

The current discharge process is often slow

and inefficient, leading to increased patient

discharge time and readmissions. Multiple

factors contribute to this problem, including

administrative tasks, communication

barriers, and inadequate patient education.

There is a need to develop a system that

addresses these challenges and streamlines

the discharge process to reduce patient

discharge time.

1.3 Objectives:

The objectives of this project are:

To develop a patient discharge system that

standardizes the discharge process and

improves communication between healthcare

providers and patients.

To evaluate the effectiveness of the patient

discharge system in improving patient

outcomes,

including reducing readmissions and adverse

To identify barriers and facilitators to the

adoption and implementation of the patient

discharge system.

1.4Methodology:

This project will use a mixed-methods

approach to develop and evaluate the patient

discharge system. The first phase will

involve a review of the literature to identify

best practices in patient discharge and

inform the development of the patient.

1.5 Expected Results:

The expected results of this project are:

1.6 Conclusion:

Reducing patient discharge time is critical

to improving patient outcomes and reducing

healthcare costs. This project aims to

develop a process improvement plan that

streamlines the discharge process and

reduces patient discharge time, leading to

improved patient outcomes and increased

healthcare efficiency.
