    POST OP NOTE

    SURGERY: Open Appendectomy

    INDICATION: Appendicitis

    SURGEON: Dr

    ASSISTANT: Dr

    ANAESTHETIS: Dr

    ANAESTHESIA: Spinal

    INTRAOP FINDINGS: Inflammed appendix

    EBL: Negligible

    PROCEDURE:

    Spinal anaesthesia was given after routine skin prepping and draping.
    Lanz incision made after McBurney’s point was identified. Incision was deepened down the
    subcutaneous fat.
    Fascia was separated, external oblique muscle bluntly dissected. Peritonuem was entered.
    Appendix was identified, mesoappendix containing the appendiceal artery was then ligated and
    separated from the appendix.
    The appendix was removed with the stump buried with purse-string.
    Layer wise closure was done.
    Skin repaired with Subcuticular stitches with Vicrl2/0.
    Procedure was well tolerated.

    POST OP ORDER

    NPO till review


    Monitor Vital signs closely
    Strict Input and Output Monitoring
    IV Ceftriaxone 1g 12hrly
    IV Metronidazole 500mg 8hrly
    IV PCM 600mg 8hrly
    IM Pentazocine 60mg 8hrly
    IM Diclofenac 75mg 12hrly
    Stagger Analgesics
    Post op PCV tomorrow
    Send Samples for histology
