Part of the socio - spiritual development program of the University of Perpetual Help System-DALTA is the integration of the value
of Health and Ecological Consciousness in all FCL classes. This is part of our University’s mission and vision of strengthening the
moral fiber of the Filipino Christian Leader as Real Helpers of God.

In this regard, The FCL 4208 classes having Community Extension Activity which is Tree Planting. This is in line with the
University principle, Character Building is Nation Building which is a vital ingredient in nation building through system of service in
health, education, technology, research and community outreach program. Below are the details of the one-day activity.

Places : Barangay San Francisco, Lipa, Batangas City

Date of Tree Planting Activity : April 20, 2023, THURSDAY

Community Outreach Program

Transportation Fee : FIVE HUNDRED PESOS (Php 500.00) ONLY

Assembly Area : University Lobby at 5:00 A.M.

Departure Time : 5:30 A.M. after registration and orientation

End of COP : 5:00 pm

Expected arrival at the

Las Pinas campus : 8:00 pm

NOTE: Kindly sign the parent’s consent below and pay the amount of FIVE HUNDRED PESOS (Php. 500.00) ONLY at the
Cashier’s Office only on or before April 17, 2023. Submit the Parent’s Consent together with the Official Receipt of payment to
your respective class leader.

in TREE PLANTING ACTIVITY at Barangay San Francisco, Lipa, Batangas City, amounting FIVE HUNDRED
(Php.500.00) PESOS ONLY and having understood that every precaution will be taken by the faculty coordinator
concerned to ensure the safety of your son/daughter during the entire activity. I shall not hold any of the faculty
coordinator nor the school responsible for any untoward incidents that may happen beyond their control.
